List of documents required for registration of Aadhaar form

The list of documents required for registration of Aadhaar form:

• Filled Aadhaar Enrolment Form
• Proof of Identity (PoI) document
• Proof of Address (PoA) document
• Date of Birth (DoB) document

Apart from the Aadhaar form, you need original or attested photocopies of rest of three documents. Any gazetted officer/notary officer can verify the photocopies.

  

A. Relevant Proof of Identity (PoI) Documents Showing Name and Photo (either of these):

• Arms License
• Binding Documents
• CGHS / ECHS Photo Card
• Driving License
• Freedom Fighter Photo Card
• Government Photo ID Cards/ service photo identity card from Public Sector Undertaking
• Kissan Photo Passbook
• NREGS Job Card
• PAN Card
• Passport
• Pensioner Photo Card
• Photo Bank ATM Card
• Photo Credit Card
• Photo ID provided by Recognized Educational Institution
• Ration/ PDS Photo Card
• Voter ID

 Address Card that bears the Name and Photo authenticated by Department of Posts

Certificate of Identification showing the photo listed byGazetted Officer or even the Tehsildar right on letterhead

Medical certification of the Disability ID Card/handicap, which is issued by corresponding State/UT Governments/Administrations

B. Relevant Proof of Address (PoA) Documents having Name and Address (either of these):

• Address Card with Photo dispensed by Department of Posts
• Address Certification with photos issued either by MP or MLA or Gazetted Officer or Tehsildar right on the official letterhead
• Arms License
• Bank Statement/ Passbook
• Bill for the Gas Connection (not 3 months old)
• Caste and Domicile Certificate with Photo issued from respective State Govt.
• Certificate of Address issued by Village Panchayat head or its equivalent authority (for rural areas)
• CGHS / ECHS Card
• Credit Card Statement (not older than 3 months)
• Disability ID Card/handicapped medical certificate given out by the respective State/UT Governments/Administrations
• Driving License
• Electricity Bill (not older than 3 months)
• Freedom Fighter Card
• Government Photo ID cards/ service photo identity card issued by PSU
• Income Tax Assessment Order
• Insurance Policy
• Kissan Passbook
• Letter Signed and bearing the Photo. The letter is signed by Recognized Educational Instruction on official letterhead
• Letter Signed and showing the Photo that is issued from the registered Company on official letterhead
• Letter Signed andshowing the Photo from Bank on the official letterhead
• NREGS Job Card
• Passport
• Passport of Parents(if the individual is a Minor)
• Passport of Spouse
• Pensioner Card
• Post Office Account Statement/Passbook
• Ration Card
• Receipt of Property Tax (only 3 months)
• Registered Sale / Lease / Rent Agreement

D. In lieu of Proof of Relationship (PoR) Documents explicitly showing the Relationship details to Head of Family (either of these):

• Army Canteen Card
• CGHS/State Government/ECHS/ESIC Medical card
• MNREGA Job Card
• Passport
• PDS Card
• Pension Card

Birth Certificate from the Registrar of Birth, Municipal Corporation and many of the notified local Government bodies such as the Taluk, Tehsil etc.

Miscellaneous Central/State government from the family entitlement document.
